Turn one long job into many, many, many small jobs. This is counter intuitive, but because the job usually takes less than a day, it means that the dwarf who did the job is close enough to haul the thing that they made. Set up manager jobs to automate things and only build one thing per job. For example, for wood, only allow dwarfs that live near the forests to haul wood. For certain hauling jobs, only turn it on for certain dwarfs. Assign workshops to individual dwarfs to ensure that they are usually hanging out in the same place and *never* walk half way across the map to weave some cloth, for instance.
Try to put workshops close to where dwarfs live. Set up traffic routes to ensure that dwarfs don't bump into each other and repath. Make sure all your main walkways are at least 3 wide (and even wider for large fortresses). Link stockpiles and workshops so that you know exactly how the pathing is going to work. Also, you probably don't want large central quantum stock piles. This means setting up *many* intermediate stockpiles. If you have to haul something, try to make sure that you never haul it more than 20 tiles, for instance. Basically try to make your paths as short as possible. All of the things you mentioned help a *little* but pathing is orders of magnitude more. I honestly believe that the vast majority of FPS death problems are pathing issues.
